Respite Care to Transitional Housing

We know that recuperation and rehabilitation are not just limited to the boundaries of our facility. That’s why we’re dedicated to giving our patients all the help they need to live fully realized lives even after they’re no longer a patient with us. We maintain strong, collaborative relationships with case management professionals to whom we refer patients once they no longer require our services. These case management professionals will then be able to assist patients to find temporary-to-permanent housing, specialized service providers, employment assistance, charity programs, and other resources that can help break the cycle of homelessness.

What is Transitional Housing?

Transitional Housing refers to temporary-to-permanent housing for homeless individuals following their stay at a recuperative care facility, such as Horizon.  Transitional housing assistance plays a vital role in breaking the cycle of homelessness and preparing individuals for their best-realized life.

Why is Transitional Housing Assistance Such an Important Service?

Horizon Recuperative Care’s Transitional Housing Assistance Program is a service that we are dedicated to offering our patients.  Recuperative care (also known as medical respite) provides the homeless with the comprehensive health care that they need following inpatient care at an acute care facility.  Once a patient is ready to leave our recuperative care facility, we firmly believe that our responsibility to their recuperation and rehabilitation does not stop there.  Without transitional housing assistance, many individuals will find themselves displaced once more as they do not have the tools they need to take the next steps towards a better life.  

How does Horizon Recuperative Care assist with Transitional Housing?

Horizon maintains excellent relationships with case management professionals to whom we refer patients once they are mentally and physically ready to leave our care.  These case management professionals assist patients to find temporary-to-permanent housing, specialized service providers, employment assistance, charity programs and other resources to help them achieve successful lives.
