As part of the California Advancing and Innovating Medi-Cal (CalAIM) program, we are working to improve health outcomes for all Californians. CalAIM is a state initiative that aims to improve access to quality care, increase coordination between providers, and reduce costs.
Through CalAIM, we are working to build a system of recuperative care with a focus on prevention and wellness. We are also working to improve access to care for vulnerable populations, including the homeless.By working together, we can make sure that all Californians have the opportunity to live healthy lives.
The CalAIM Recuperative Care Program provides short-term skilled nursing care, rehabilitation therapies, and other services to help individuals who are discharged from a hospital stay recuperate at home or in another community setting. The program is designed to improve health outcomes and reduce readmissions by providing coordinated, wraparound care that meets the unique needs of each individual.

In addition, the CalAim Community Supports program is designed to improve access to quality care and service by providing funding for community-based organizations to provide case management, housing assistance, and other services to help people stay healthy and independent.
The program is currently being piloted in Los Angeles. The program is open to all Medi-Cal beneficiaries who are discharged from a hospital stay and meet the program’s eligibility criteria. For more information about the CalAIM Recuperative Care Program, please contact us.
